LECOM’s Office of the Registrar is committed to serving current LECOM students, alumni, faculty and staff at all campuses.

It is the registrars responsibility to:

  • Provide and maintain transcripts for current students and alumni
  • Provide enrollment updates and degree verifications
  • Complete state licensing forms including applications to ERAS
  • Complete name and address changes

This office also operates in accordance with the Family Educational Rights and Privacy Act of 1974 (FERPA), protecting individual rights and privacy.

Transcript Request

LECOM does not issue unofficial transcripts.  A fee of $10 is charged for the issuance of each transcript. An official transcript is ordinarily mailed directly to other institutions and agencies.  Because official transcripts have an embossed seal, they cannot be faxed or e-mailed. Whenever a transcript is released directly to a student, it bears the stamped message “Student Copy.”

Please allow five business days for processing after the request is received.   Incomplete or illegible forms may delay processing.

Transcript Request Instructions:

  • Download the Transcript Request Form
  • Print out the form
  • Complete the form (Please print legibly)
  • Include signature and date (Do not use a computerized font for your signature; it will delay processing because your original written signature is necessary)

LECOM does not keep copies of graduates’ diplomas. Graduates may send a copy of their diploma to the Registrar’s Office for certification. Requests for name changes taking place after graduation incur a fee. Further details can be found in the current Academic Catalog and Student Handbook. Requests for reprints of diplomas (without name changes) should be directed to erie-registrar@lecom.edu.

Current students will receive an ERAS Token for Residency during their third year. Any graduates in need of an ERAS Token for Residency will need to submit a $75.00 check to the Registrar’s Office, payable to LECOM, for re-application to ERAS. Please email the Registrar’s Office for further information at erie-registrar@lecom.edu.

LECOM graduates applying for Fellowship through ERAS/MIDUS can submit their Transcript and MSPE requests online, through their ERAS account. There is no transcript fee or transcript request form needed. Once the contact information below is submitted for both the transcript and MSPE, the Registrar’s Office will upload these documents directly to the graduates’ ERAS account. Current Students/Graduates – The Registrar’s Office processes requests for letters of enrollment, degree verifications, and letters of academic standing. Please complete the Enrollment Verification, Letter of Good Standing Request Form with all information completed, including your signature (computerized fonts are not accepted). Please note that we are unable to address any letters “To Whom it May Concern.”

Third-Party Requests – Any employers or third-party companies requesting an education verification must email or fax their verification request to the Registrar’s Office along with a signed release form from the student/graduate. Failure to provide these documents may result in the delay of the request.

Licensing Verifications – Students/graduates applying for State Licensure may submit to the Registrar’s Office the state’s Verification of Medical Education form (applicable to the state in which you are applying). These forms can be sent via mail, email, or fax. Once received, the form will be completed and sent as directed on each state’s form.
